    • Pre-op labs when liver issues aren’t suspected.                                                                                 Example: A patient with hypertension or dehydration, a BMP is sufficient.

 

Electrolyte Panel – Waived (if not broken apart)

Quick check of electrolytes only.

  • Kidney function is already known or will be checked separately.

  • Common for:

    • Monitoring IV fluids or diuretics

    • Acid-base disorders

    • Postoperative or critical care monitoring

    • Short-term electrolyte replacement therapy

             Example: A patient on IV fluids or diuretics (e.g., furosemide) — you just need Na⁺, K⁺, Cl⁻, CO₂ to guide           

             replacement.

 

Renal Panel – Waived (if not broken apart)

Check both electrolytes and kidney function (BUN/creatinine).

  • Common for:

    • Chronic kidney disease (CKD)

    • Acute kidney injury (AKI)

    • Hypertension or diabetes

    • Nephrotoxic medications (e.g., ACE inhibitors, NSAIDs, antibiotics)

    • Dehydration or fluid overload

    • Preoperative or baseline labs

            Example: A patient with decreased urine output or elevated creatinine in the past — order a renal panel to   

            assess kidney function and electrolytes together.

Opioid (partial agonist) cut off 10ng/ml. Used for opioid dependency (Suboxone®). Not detected by standard opiate screen; needs its own assay. Detection Window 2–6 days

Secobarbital (BAR) 300 ng/mL

Barbiturate 300ng/ml cut off Sedative/hypnotic. Cross-reactivity among barbiturates. Detection window 2–10 days

Oxazepam (BZO) 300 ng/mL

Benzodiazepine 300 ng/ml cut off Detects diazepam, temazepam, oxazepam. Lorazepam may not cross-react—know limitations. Detection Window 1–7 days (longer for chronic use)

Nortriptyline (TCA) 1000 ng/mL

Tricyclic antidepressant 1000ng/ml cut off TCA overdose risk; screen detects many TCAs. Confirm if positive. Detection window 1–10 days

Marijuana/THC 50 ng/mL

Cannabinoid 50ng/ml cut off Detects THC-COOH metabolite. Chronic use prolongs detection. Passive exposure rarely exceeds cutoff. Detection window 3–30+ days (depends on use)

Fentanyl (FYL) 1 ng/mL

Synthetic opioid 1ng/ml cut off Potent opioid; not detected by standard opiate screen. Specific fentanyl assay required. Confirm positives with LC-MS/MS. Detection window 1–3 days
